Instructions for Initial Visit
Patients on their initial visit should fill out the following forms prior to your visit. This will allow the maximum time for their treatment.
Be sure to bring the completed forms with you to your visit.

Initial Evaluation: about 1 1/2 hours
Follow up and maintance Visits: 45 minutes to 1 hour
24 Hour Cancellation Notice Required
Costs:
Initial Evaluation: $100
Follow up and maintance Visits: $70
Please make arrangements to pay for your visit on the date of service. We regret we are unable to process insurance claims.
